Patient Centered Medical Home
We are proud to state that Rocha Medical Clinic has achieved the level 3 PCMH Certification of NCQA.
This is a clear measure of the quality and services provided to our patients under the medical home concept.
Dr Rocha's office is a member of only a few practices in San Antonio that have achieved the level 3 certification under
the NCQA- PCMH certification.
Dr Rocha has participated in the quality improvement process for years. He has served as chairman of the quality committee in Santa Rosa Hospital and Southwest General Hospitals in the past and has been member of the quality committee at Baptist Medical Center and the regional Tricare quality committee in the past.
We at present participate in PPR Net for the reporting of quality measures in primary care practices, and actively involved in the reporting of quality measures for Humana Gold and Rio Grande Valley ACO. Dr Rocha also serves as a board member of the RGV ACO, the top ACO in the nation.
What's an ACO?
Accountable Care Organizations (ACOs) are groups of doctors, hospitals, and other health care providers, who come together voluntarily to give coordinated high quality care to their Medicare patients.
The goal of coordinated care is to ensure that patients, especially the chronically ill, get the right care at the right time, while avoiding unnecessary duplication of services and preventing medical errors. When an ACO succeeds both in both delivering high-quality care and spending health care dollars more wisely, it will share in the savings it achieves for the Medicare program.
Medicare offers several ACO programs / models:
- Medicare Shared Savings Program — a program that helps a Medicare fee-for-service program providers become an ACO. Apply Now.
- Advance Payment ACO Model — a supplementary incentive program for selected participants in the Shared Savings Program.
- Pioneer ACO Model — a program designed for early adopters of coordinated care. No longer accepting applications.
